The Community Care Vans currently offer screening, education and medical management related to hypertension and diabetes. The team provides walk-in-visits for the following: • Blood pressure checks and care • Adjustment of hypertensive medications as needed • Written recommendations for follow-up – blood pressure care plan • Provision of BP cuffs and education about their use • Diabetes education and A1C assessment • Social need assessment and intervention • Education related to exercise and nutrition • Substance Use Disease (SUD) Care (at specific clinics)All free.
